I was a mechanic in a local shop here and most of the problems that occurred with that engine was caused by the controller in the injection pump, that pump should have a round controller with two wires that pushon from the top, the wires also have 90degree terminals on them comes in from the front of the injection pump, but most of time it would act like what you are describing on the rpm not revving up or surging, but sometimes didnt log a code, I would pursue the fault code first and if you dont come up with anything I would look at the injection pump controller, I have also replaced the ecm which is mounted on the injection pump, black rectangular box about 4 inches wide by 6 inches tall about 1 inch deep has a rectangular wiring harness going in with 2 small screws going in, there is not a good trouble shooting for these items, if you need more info email me and I can tell more of what I know, I have had guys tell me that they pulled the controller out and cleaned it and fixed it, Cummins has connectors to replace the ones on harness cause they lose contact they will feel like they are tight but it is the plastic holding tight and not the electrical connector, Cummins wasnt very fond of that system, I have replaced those pieces and last resort and fixed them, I am not sure what price they get for them now but they werent something you just wanted purchase to have around. This may not help but keep it in mind when you are running them, if you get alot of fault codes the battery connections are very important to BE CLEAN not just look clean this doesnt sound like it is the problem though
