This is not an isolated problem on the S series combines.

Mine was the third one my dealer has seen this year.

The idler sprockets have hexagon bores riding on hexagon sleeves.
The sleeves are wearing in the bearings, the sprocket will move sideways and get stuck on the worn portion of the sleeve.

Mine is a 2012 with a 50 roller chain, the later S series have a 60 chain, no
improvement, still comes off.
